Faculty Licensure Renewal

App State faculty may request assistance with the Professional Educator Licensure Renewal process through the Office of Field Experiences. Once your request is received, you will receive an email containing the steps and required forms to complete your renewal.

Licensure Renewal Instructions

1. Review Renewal Requirements

Before beginning, review the renewal requirements on the North Carolina Department of Public Instruction (NCDPI) website:

2. Complete Form U

  • Superintendent/Designee: Dr. Monica Lambert, Associate Dean for Educator Preparation and Student Affairs
  • Ensure all required fields are completed.

3. Complete the RCOE Faculty Licensure Renewal Form: Activities

  • This form requires Associate Dean, Dr. Monica Lamberts' signature.
    • Make a copy of the form, save it as a PDF, relabeling it using the following format:
    • LastName_FirstName_Licensure Renewal  
      • *Example: Doe_Jane_Licensure Renewal

4. Submit All Renewal Document Materials 

  • Create a single PDF of the Form U and RCOE Faculty Licensure Renewal Form Activities Google sheet. 
    • Any additional supporting documents accompanying the renewal forms should be saved as a separate PDF format: LastName_FirstName_Licensure Renewal  
    • Make a copy of your Google Docs and Sheets for your records.
  • Email your saved PDF renewal documents and any supporting (CEUs, Conferences, etc.) materials to:
  • The Administrative Support Specialist will forward your materials to Dr. Monica Lambert for review and signature. And once the forms have been signed, she will return the completed documents to you to proceed wth you renewal application.

5. Submit to NCDPI

Submit the signed Form U and RCOE Faculty Licensure Renewal Form: Activities, along with your application through the NCDPI Online Licensure System.
